Ride & Attraction Waivers

Funplex Amusement park requires a signed waiver in order to participate i some of our more physically-involved park activities. You can find these forms below. If you or your child will be participating in our Go-Kart or Roller Skating activities, we ask that you fill out the appropriate form(s), and bring them with you when you come to visit the park.

After you've submitted a waiver once, you won't need to submit it again on your following trips to the Funplex.
